Acute pancreatitis for several years consistently ranks second, and in some regions - first place in the structure of the urgent surgical pathology, ahead of acute appendicitis and acute cholecystitis. The most common cause of disease is alcohol abuse and gallstone disease in 10-30% of patients the etiology of acute pancreatitis remains unclear. Basis of the study were the results of examination and treatment of 136 patients with destructive forms of acute pancreatitis, enrolling for treatment at the Clinical Hospital № 23 them. «Medsantrud» in the period from 2008 to 2010 for acute pancreatitis. In the studied group did not include the postoperative biliary and infected pancreatic necrosis because of features of pathogenesis. Differentiated approach to surgical treatment in patients with destructive pancreatitis depending on the scale of the initial lesion of the pancreas has allowed an average reduction in the number of purulent complications per patient in 4 times (from 9,8% to 2,2%), lower letalnost of purulent complications 3 times (from 15,3% to 5,5%). The tendencies of high incidence of ocular organs particularly in children and teenagers have been investigated. Growth of incidence leads to an increase of oph-thalmological medicines (OMs) use. We analyzed the deaths of HIV-infected basing on materials of SHI (state health care institution) Novgorod Regional Infectious Diseases Hospital during 2004-2010. The main cause of death was generalized tuberculosis, chronic viral hepatitis in cirrhotic stage, late admission. Новг. гос. ун-та. Сер.: Медицинские науки. 2011. №62. С.8-10. We studied the relation of physical health of students in the post pubertal period with the peculiarities of their development. It is shown that the maturation of schoolchildren accom-panied by an increase of the index of physical health (IPH). It is proved that the health of school children is dependent on gender, age and body mass index (BMI). Physical development of boys ahead of such girls. It contains amber acid, riboxin, nicotinamide, riboflavin mononucleotide sodium. CFL has antioxidant and antihypoxic action, positive effect on energy production in cells, reduces the production of free radicals and stops the activity of antioxidant enzymes. Сер.: Медицинские науки. 2011. №62. С.91-92. We observed 78 patients with false aneurysms of the distal anastomosis after previous reconstructive surgery in aorto-femoral segment. Of the 78 operated patients good immediate result was noted in 65 patients (83.3%), amputation was carried out in 9 (11.5%) cases. Postoperative lethality was 5.1% (4 patients). The experience of modern radiation techniques of diagnosis was assessed in 668 patients with primary multiple tumors and proliferative processes of the breast. 97 patients had bilateral breast cancer, 77 — multiple and 49 — metastatic cancer of second breast. The rest women have been diagnosed different kinds of proliferative diseases and cancer at different stages, even carcinoma in situ by the complex radial and morphological research. The implementation of radiological methods at dispensary check-up allowed improving the quality of the clinical examination of patients with breast cancer and increase of bilateral forms in the Novgorod region over the past 5 years on 2.15%. In 4.3 ± 2.1% of women clinical radiodiagnosis was uninformative or misleading. Necessity of morphological verification of primary multiple tumors of unknown origin has appeared. We developed less invasive way to remove the focal formation by special device with cryoapplicator under ultra-sound control, which is promising approach to clarifying diagnosis and at the same time it is the method of medical prevention of invasive forms of breast cancer. There were analyzed the data given by different authors about the frequency of occurrence, specific pathogenesis, clinical progression of duodenal ulcer in patients of elderly and senile age. In the pathogenesis of peptic ulcer in elderly age alongside infectious link attenuation of the protective mechanisms of the mucous membrane of the stomach and duodenum plays a major role. The complex survey of 80 AIM patients was conducted. The first group included EPIA III C class 60 patients with high and intermediate risk, according to E.Braunwald (1994) classification. For comparison it was made the group of 20 patients with uncomplicated macro focal IM. Survey of autonomic nervous system condition and nitric oxide concentration was conducted. Results testify that at early postinfarctial stenocardia patients detected expressed contraventions of all links of autonomic nervous system, appeared by rather low sympathetic tone with excessive reactance. At acute myocardial infarction complicated with early postinfarctial angina patients the histic adrenergic receptors’ density is increased and simultaneously paradoxical increase of nitric oxide’s metabolites concentration. Disorder of homeostasis and blood aggregation in patients with obstructive jaundice of non-neoplastic ethiology // Вестн. Новг. гос. ун-та. Сер.: Медицинские науки. 2011. №62. С.60-63. Indicators of homeostasis have been studied in 457 patients with obstructive jaundice. 232 of them had cholestasis for 10 days and 225 — for 3-6 week. Indicators have been studied before and after the membrane, antioxidant and anti-platelet therapy in 140 patients. Increase in viscosity, erythrocyte aggregation, decrease of their deformability, blood coagulation potential and increase of fibrinolytic activity of blood have been observed in patients with obstructive jaundice on the background of disorder of lipid metabolism and endogenous intoxication. All these changes are more manifested in patients from the second group with prolonged obstructive jaundice when cytolytic processes progress along with cholestatic ones. In the framework of evidence-based medicine an attempt was made to assess the effectiveness of comprehensive prevention of early pregnancy to restore the normative parameters of the uteroplacental hemodynamics and cytokine level, morphological and functional characteristics of the placenta and afterbirth. The results of a comprehensive preventive medicine using magnesium lactate and chloride, pyridoxine, folic acid, essential phospholipids (substance EPL), glitsirrizirovannoy acids and probiotics to functional and morphological changes in the placenta, and clinical outcomes were estimated. 53 pregnant women at high risk were examined. The results showed that, despite high levels of placental pathology, an integrated drug treatment improved outcomes of pregnancy and the state of the fetus at birth. Библиогр. 13 назв. The features of remodeling of the left and right ventricles of the heart in patients who died suffering from arterial hypertension has been studied basing on data from postmortem autopsy of 11 years in Semey. 372 autopsy protocols have been selected. Analysis of protocols showed that left ventricular hypertrophy was observed in almost all cases, while right ventricular hypertrophy in 78.2% of the cases. Amongst 305 patients with EC-cellular morbid growth of gastrointestinal tract, 21(6,8%) have been diagnosed with pancreas carcinoids.There are certain difficulties, associated with diagnostics and treatment of neuroendocrinal oncomas in this focalization. One of the clinical observations exemplifies difficulties of verification pancreas carcinoids and deficiencies of medical assistance arrangements, even when conducted by specialised medical services. It is demonstrated that laboratory equipmentaided examination methods in the pancreas carcinoids diagnostics are applied in a format of general examination concepts, same as used for abdominal cavity illnesses. Pancreas carcinoid post-operational therapy selection is made without taking into account specificity of clinical presentations of illness, relevant morphological research data or EC-cellular morbid growth dynamics prognosis. By use of data, obtained from domestic and international publications sources, possible ways of improvement of medical assistance quality in cases of pancreas carcinoids are being given consideration in regards with both the organisational and technical aspects (of this problem). Results of morphological research of marginal gingival tissues before surgical treatment and in process of wound healing in patients with traditional postoperative period and application of transcranial stimulation are presented. For the undergraduate students in the course of a long observation period (one academic year) the test study has investigated the temporal dynamics of the personal-level determinants of purposeful behavior — achievement motivation, and cardio-respiratory support of its implementation in the emotionally intense periods of learning activities in terms of using a rating system of knowledge assessment system and pass/fail. It is shown that a stable low level of AM (achievement motivation) provided different versions of intersystem relations of cardiovascular and respiratory functions under-rated and un-rated system of education. The paper presents data on 371 patients who had undergone hip arthroplasty. The adopted in the hospital algorithm of aged patients management, for whom hip arthroplasty is necessary, meets the requirements of Federal Standard and allows for such intervention after a comprehensive survey with minimal risk to patients. The analysis of mortality from major causes — cardio-vascular diseases, malignant neoplasms, external causes, which occupy in the structure of the total mortality over 85% in the Novgorod region in the first decade of the XXI century, is presented. The differences in levels of mortality compared with the Russian Federation and the Northwestern Federal District are found out. Since 2004 stable trend of reducing mortality and increasing life expectancy has developed. Despite this, mortality remains high. It is emphasized that significant reduction in levels of both total mortality and the mortality from main reasons may be achieved primarily due to changes in individual human behavior. Basing on the survey of 259 patients with essential arte-rial hypertension (EAH) early clinical diagnostic markers of involvement in the pathological process of the kidneys have been defined. The survey, which included assessment of micro-albuminuria, glomerular filtration rate, investigation of indicators of renal hemodynamics by Doppler method and study of urinary excretion of molecular mediators that reflect patho-physiological processes in the kidney in patients with EAH, helps to set the dynamic characteristics of early hypertensive nephropathy and to define complex of clinical diagnostic markers of the stages of evolution of hypertensive vascular nephropathy. The earliest signs of involvement in the pathological process of the kidneys in patients with EAH are increasing of urinary biomarkers of endothelial dysfunction closely associated with its remodeling of the extracellular matrix of the kidney and microalbuminuria. We can assess the progression of hypertensive kidney disease with the formation of maladaptive remodeling of intrarenal vascular and growth of ischemia of renal kidney by increase of resistance index of interlobar renal arteries, the downward trend in GFR (less than 90 ml/min/1.73 m2), along with increase of urinary excretion of markers of fibrogenesis in the kidney, particularly of collagen of IV type in EAH patients with persistent microalbuminuria. It was noted that the decrease of GFR less than 60 ml/min/1.73m2 is recognized marker of subclinical dysfunction of the kidney in patients with EAH, it is first evidence of the development of chronic kidney disease in these patients that allows to relate this category of patients to group of high risk of progression of hypertensive kidney disease and requires active nephroprotective strategy. Pathomorphological comparison of viral liver cirrhosis (induced by hepatitis B virus, hepatitis C virus or both) and viral-alcoholic liver cirrhosis is presented. It was shown that patients with viral-alcoholic cirrhosis decease in younger age (p = 0,004). The article is based on study and systematization of causes for death of 71 patients among 304 patients having acute destructive pancreatitis. Active surgical tactics was applied for the control group of patients (138), in the basic group of patients (166) the tactics of treatment was optimized by means of use of new technologies. The main principle for the basic group was refusal of early traditional operative interventions that led to reduction of fatal outcome more than a half as much (2.1). Early hospitalization and timely beginning of treatment, age and presence of associated diseases, volume of pancreatonecrosis, involvement of distant areas of spatium cellulosum, amount of systematic complications and optimized algorithm of treatment influenced fatal outcomes. The components of electrical discharges are directly acting and destroying the bacteria cell inside the liquid and we extensively used this factor in contaminated surgery. The experiment in vitro shown the inhibition of biofilm formating ability of bacteria under the impaction of a subinhibiting dose of IED. This effect is more pronounced for gram negative bacteria. The results showed an independent contribution of the parasympathetic nervous system in the development of structural changes in the myocardium. Changes of the extracellular matrix are more characteristic for the cholinergic stress , particularly an increase of extracellular spaces and to a greater extent in the right ventricle of the heart. Effective therapy with amlodipine reduces the load on the target organs by elevated systolic blood pressure, which also increases the load by lowered blood pressure, especially diastolic BP at night, and to a greater extent in hypertensive patients with subclinical depression. BP variability is higher in hypertensive women with subclinical depression during the day.
